Add or remove a dependent

UATP members who have an Insider's Club or Executive Connection family plan in-force may choose to include coverage for their Spouse and/or Dependent Children. Your spouse and/or dependent children can be added or removed online at the time of re-enrollment. To add or remove a spouse or dependent children at any point during your current plan term, please call our office at 1-800-383-0049 or use our secure online Contact Us form.

Children/Child means a person under the age of 25 and primarily dependent on You for support and maintenance. The age limit does not apply to a child who is incapable of self-sustaining employment by reason of mental or physical incapacity.

Update your coverage (when you are re-enrolling your current Insider’s Club or Executive Connection family membership, with no changes to your plan type) by specifying your Spouse and/or Dependent(s) below. The designation(s) submitted below will overwrite any Spouse or Dependent(s) specified previously under your plan.

If you are adding a spouse or dependent child(ren), I.D. cards will be sent to these individuals within 7-10 business days.
